Out of Network Insurance
If you do not see your insurance company listed above- you can still receive services from us, however you would be billed directly at time of session in full at our self pay rate.
If you wish to submit for reimbursement to your insurance provider, Polaris Counseling and Wellness, LLC will provide necessary documentation to assist. However payment and submission of documentation is responsibility of the client.
Discuss with individual provider, as potential for Polaris Counseling and Wellness, LLC to seek future in-network status on your behalf for future sessions. However, you would be responsible for all costs/seeking out of network reimbursement, until an agreement was signed by your insurance provider and Polaris Counseling and Wellness, LLC.
No Surprises with Billing/Costs
From intake, you will be kept informed of any potential payments or bills for services rendered. As part of intake paperwork you will receive a full copy of all estimated costs of all services offered at Polaris Counseling and Wellness, LLC.
Most clients are seen weekly or bi-weekly depending on treatment needs. To protect clients from large bills payment is due at time services are rendered. Full Copy of our fee schedule is provided upon intake.
For in network insurance payments, you are only responsible for out of pocket expenses as outlined by your insurance coverage. This is determined by your covering insurance and not determined by our practice. We promote you to outreach to your insurance company to confirm coverage with an identified provider and ensure understanding of your payment expectations/insurance benefits. *Any changes with insurance during treatment are required for informing provider before session to avoid any unexpected cost for services rendered*
For out of network, you will be billed directly for self-pay rate and promoted to submit claims independently to your insurance to seek reimbursement. We will ensure to give you all documentation needed to be able to submit a claim. Typical self-pay rate for sessions is dependent of the provider you are seeing. Typical self pay range is between $120-$165 per 60 minute session.
For Self Pay, we have set rates for all services however we do offer sliding scale that is determined through household income and eligibility. This offers time limited discount sessions. Application is completed through our patient portal with proof of income reviewed.
At any point in time, the client holds the right to discontinue services.
